Short Description of this Case
The class-action lawsuit, filed in July, 2008, alleged that Arizona United Healthcare improperly denied coverage for nonsurgical spinal decompression therapy treatments and failed to pay therapy reimbursement, which violated the Employee Retirement Income Security Act (ERISA).
There are 2 groups of class members:
Group 1: Class member who is a Plan Member who paid to a medical provider any portion of the amount billed by a medical provider above copayment, coinsurance, or deductible for a Complete Claim Denial of Out-of-Network Decompression Therapy Services received between March 7, 2002 and April 24, 2019.
Group 2: Class member who is a medical provider who provided Out-of-Network Decompression Therapy Services to a Plan Member for which a Complete Claim Denial was received between March 7, 2002 and April 24, 2019.
